Section 3 Health and Safety at Work Act 1974
With reference to the above Act, this form provides relevant information regarding some of the risks involved with some of the hair & beauty procedures available at Lerage. An appropriate pre-treatment skin test, prior to each beauty procedure, is recommended for health reasons as well as for client comfort and safety. It is the responsibility of the client to plan and receive a complimentary pre-treatment skin test at Lerage, Regis, 25 Corporation Street, Birmingham B2 4LS and within the recommended period stated below

- It is the responsibility of the client to plan and receive a complimentary pre-treatment skin test, within Lerage, at least 48 hours ahead of any beauty procedure that may cause an adverse reaction (such as waxing, hair tinting and fake tanning). It is also the clients responsibility to retain evidence of The Test i.e., date, time, products used and area tested, as provided by a Lerage professional
- It is the responsibility of the client to plan and receive a complimentary pre-treatment skin test, within Lerage, no longer than 7 days ahead of any beauty procedure that may cause an adverse reaction (such as waxing, hair tinting and fake tanning). It is also the clients responsibility to retain evidence of The Test i.e., date, time, products used and area tested, as provided by a Lerage professional
- A pre-treatment skin test is a precautionary measure intended to reduce, although not deemed to remove, the risk of an adverse reaction both during &/or following a skin test or more intense exposure
- It is possible that a pre-treatment skin test may induce sensitisation to a substance and/or procedure on any occasion
- When a client attends an appointment to receive either an appropriate pre-treatment skin test or complete beauty procedure, the client is deemed to accept the risk of an adverse reaction
- When a complimentary pre-treatment skin test is not planned and received at this salon within the recommended period (refer to 1+2), and for whatever reason, her therapist shall deem the client to have performed an appropriate generic skin sensitivity test and within the recommended period (refer to 1+2). It should be noted that a skin test, conducted by a professional, is recommended and more likely to be accurate
- When a client books and then proceeds to receive any beauty treatment that may or may not require a pre-treatment skin test, the therapist shall deem the client as having accepted that there are risks involved. These might be, but are not limited to, the adverse reactions listed in 8+9+10
- The risk that an individual, with PPD hypersensitivity (known or unknown), takes when receiving a hair dye sensitivity skin test and/or more intense exposure is the consequential reaction such as, but not limited to, the appearance of weal, erythema, itching, redness, eczema, pain, swelling and/or burns
- The risk that an individual, with DHA hypersensitivity (known or unknown), takes when receiving a sunless tanning sensitivity test and/or more intense exposure is the consequential reaction such as, but not limited to, contact dermatitis. Most cases, however are due to other ingredients in the skin product preparation, such as preservatives, plant extracts, dyes or fragrances
- The risk that an individual, who does not follow the pre-treatment and post treatment (aftercare) instructions for wax (refer to 11), takes when receiving a depilatory wax (hair removal) procedure is the consequential reaction such as, but not limited to mild to moderate exfoliation, burns, itching, redness, swelling, scabbing, hyper pigmentation; and mild to excessive pain, erythema and bruising
- The pre-treatment, treatment and post treatment instructions (aftercare) for wax include the aforementioned pre-treatment skin test along with (1) the avoidance of hot water bathing, swimming, sauna, solarium, sunbathing, steam bathing, exercise and/or any activity that induces mild to excessive perspiration for 24 hours before and after the procedure is received. Additional post wax instructions (aftercare) are (2) the avoidance of restricted clothing in the area, (3) aftercare products, applied directly post treatment and recommended by your professional, should be applied regularly to the treated areas until the skin has returned to its normal colour, texture and sensitivity (post care products may be purchased in-store) (4) It is the responsibility of the client to request that their treatment is brought to an end at the first instant he/she considers it is necessary and for whatsoever reason, if any at all
- By signing this agreement I confirm I have read & completely understood the above instructions & advice relating to Pre-Treatment Skin Tests and Pre/Post Treatment Care
- I confirm I shall retain a copy of The Instructions & Advice and, thereafter, refer to it each time I book to receive a beauty procedure
- If I mislay The Instructions & Advice I shall immediately request a copy from Lerage
- I accept it as my responsibility to cancel my beauty booking if I fail to follow any of the pre-treatment advise contained within this information and/or, if I feel ill or highly anxious on the day that I have booked to receive a procedure
- I shall inform Lerage in writing and also retain a copy of the same letter, signed by a Lerage professional
- if I receive a black henna temporary tattoo
- if I suffer any ill health that requires medical supervision
- if I suffer or have suffered an adverse reaction to any hair or beauty procedure
- I am aged 16 or over
